The 24 Hour Reporting System is a
detailed, beneficial reporting system available to generate a priority
list of residents needing focus by health care professionals. The
nurses and managers will be able to access the resident "change of
condition" as soon as it is documented by the nurse on the unit.
The professional health managers will be able to retrieve the
information alerting them to address quality care improvement issues.
The morning report is automatically generated, eliminating staff from
writing out lists and notes as reminders. The key personnel can
create a real-time care plan update, document progress, as often as
needed, and resolve the need for 24 hour monitoring. This may be printed
for the medical record. A slideshow is on the next page with an
example of a few entities of the menu.
A
24 Hour Morning Report
As department managers gather each
morning for their morning report, they can access the summary
report on their own desktop. Many manager's arrive early before
the morning report session and will have already visited with the
resident, acted upon the resident's change-of-condition and made care plan updates.
The morning report supports an interdisciplinary approach. This
interdisciplinary approach ensures that the resident receives improved
quality of care.
Interdisciplinary Team -
The Priority List Keeps Staff Focused
The interdisciplinary team members
will now be able to enter changes that occur for each resident at
anytime, their comments for the type of change, and their approach for care plans.
The real-time report can be printed and used for the medical record
based on the care plan information, the updated progress, and resolution.
24 Hour Flow Sheet
A Flow sheet has been prepared to
show you how efficiently the program complies with meeting expectations of
the 24 Hour Reporting of changes. Only one example is show below.

Tracking Quality
Improvement
Tracking the resident's history of change-of-condition is accessible at anytime,
allowing the caregiver the ability to quickly find what was successful
in resolving the problem the last time it occurred. This is important as archived medical records will not
have to be pulled searching for previous information.
